Referral Process

1. Contact our office by phone or fax with patient information.
2. Our team reviews the referral and contacts the patient to schedule an evaluation.
3. The patient undergoes an initial consultation and assessment.
4. We develop a personalized treatment plan and communicate it to the referring provider.
5. Regular progress reports are sent to the referring physician throughout treatment.
